Atât la momentul diagnosticului CBP cât și pe parcursul evoluției bolii se pot asocia alte boli autoimune. Una dintre ele, cu afectare hepatică, fiind hepatita autoimuna (HAI), caz în care se boala se numește Overlap de CBP și HAI, având modificări pe testele serologice sugestive pentru ambele patologii. În această situație, pentru confirmarea diagnosticului este necesară și biopsia hepatică. Este important diagnosticul întrucât este necesar și alt tratament, iar fără acesta prognosticul este mai prost. 

Alte boli autoimune care se pot asocia cu CBP sunt: Sindromul Sjogren, Sindromul CREST (sindrom Raynaud, disfuncție esofagiană, sclerodactilie, telangiectazii și calcinoză), scleroza sistemică, tiroidita autoimună și boala celiacă.
